Preparedness for Seniors and Caregivers

Create a care handoff with contacts, routine needs, accessibility requirements, and backup helpers. Ask permission before sharing personal details.

Make it practical

Arrange transport that works without elevators. Discuss medical-device power and medication access with providers. Test whether the backup helper can locate supplies and follow the written plan.

Write a care handoff

  • With the person’s permission, record contacts, assistance needs, mobility requirements, and communication preferences.
  • Ask qualified providers about equipment power, refrigeration, supplies, and continuity arrangements. Store the resulting instructions with the plan.
  • Identify primary and backup helpers and transport that works without an elevator. Confirm how a destination can support the person’s needs.

Test the plan respectfully

Ask the backup helper to locate essential supplies and explain the next steps. Keep the person involved in decisions and preserve their independence where possible. Do not store detailed medical histories in a shared browser. A printed care plan should be protected and shared only with appropriate helpers. Update the plan after changes in mobility, treatment, equipment, or living arrangements.

Put this into your household plan
